A horizontal line has only 1 price coordinate - you don't need that for a horizontal line.
A trendline has 2 price coordinates - however, you also don't need that for a trendline.
A rectangle has 2 price coordinates, and the prices are different. One is the upper band, the other is the lower band of a rectangle.
So, in the case of using a rectangle, you need to know which price coordinate you want the MA to cross.
Unless you are using a rectangle like a horizontal line...
(people do strange things...)
2. MA Break Bar Index for Trendlines:
This is in case you are using a trendline at an angle.
You can experiment for yourself and see how that works.

3. Is the indicator only for MA closing above the lines or both MA closing and price candles closing?
By default the indicator looks at the first closed Bar and the second closed Bar.
So, by nature, when a MA crosses any price level, it will be on a closed Bar.
If you're looking for the Holy Grail indicator - STOP!
